How to Request Records
You can request records in two ways:
Online Form:
Please use the Fort Bend County ESD No. 4 Records Request Form provided below.Paper Form:
If you prefer, you can download and complete the Records Request Form and submit it via the methods listed below.
Your records requests are welcome, and we are committed to processing them efficiently. The Public Information Act allows governmental agencies up to 10 business days to process an open records request.
For Incident Reports:
Please use either the online form below or the paper Records Request Form to obtain a copy of an incident report.
Fees and Processing Information
Document Charges:
Documents totaling 10 pages or less will be provided at no charge. For requests exceeding 10 pages, a fee of $0.10 per page will apply.Labor Costs:
If your request requires more than one hour of personnel time, a labor charge of $15 per hour will be incurred, starting from the second hour. This fee covers the time required to locate, compile, and reproduce the requested information.Record Pickup:
You will be notified when your records are ready for pickup at the Fulshear Fire Department, Station #1, Fulshear, Texas. Please note that requested records not picked up within ten business days of notification will be destroyed.

Important Notes
- Do not submit payment with your request.
- Submit your request only once through the form, email, fax, mail, or in person.
- Please allow up to ten (10) business days for a response.
